Beyond the Tunnel View: How 360° Capsule Endoscopy Is Transforming Small-Bowel Visualization

A capsule may be small—but the perspective it provides can make a significant difference.

The small bowel has always presented a unique diagnostic challenge. Its length, complex anatomy, and location beyond the routine reach of conventional upper and lower endoscopy can make complete mucosal evaluation difficult.

Capsule endoscopy changed that journey by allowing clinicians to examine the small bowel through a minimally invasive, patient-friendly procedure. Yet not every capsule sees the small bowel in the same way.

Traditional capsule systems generally rely on a forward-facing camera, producing what can be described as a “tunnel view.” While this perspective follows the direction of travel, clinically relevant findings are often located along the mucosal wall, between folds, or around the inner curvature of the bowel.

This raises an important question:

Should capsule endoscopy look forward—or should it look directly at the mucosa?

A Different Perspective: Looking at the Mucosal Wall

CapsoCam Plus® by CapsoVision takes a different approach to small-bowel imaging.

Instead of using a single end-facing camera, the capsule incorporates four laterally oriented cameras positioned around its circumference. Together, they capture a complete 360° panoramic view of the small-bowel mucosa.

This lateral orientation allows the cameras to look directly at the bowel wall rather than primarily looking ahead through the lumen. The resulting panoramic perspective is designed to provide broader visualization of the mucosal surface, including areas that may be difficult to observe with conventional end-viewing technology.

CapsoCam Plus is currently the only capsule endoscopy system offering a complete 360° panoramic view of the small bowel.

Four Cameras. One Complete View.

Inside the compact capsule is an advanced imaging system designed to capture the small bowel from every direction.

CapsoCam Plus features:

·        Four laterally oriented, high-resolution cameras

·        A complete 360° panoramic field of view

·        Sixteen independently controlled LED lights

·        Up to 20 frames per second—five frames per second per camera

·        Smart Motion Sense™ technology

·        Onboard data storage

·        A typical battery life of approximately 15 hours

·        A capsule size of approximately 11 × 31 mm

Together, these features support detailed imaging throughout the capsule’s journey while reducing unnecessary or repetitive frames.

The longer battery life is particularly valuable during prolonged small-bowel transit, helping support the possibility of completing the examination even in patients with delayed gastrointestinal motility.

Why Panoramic Visualization Matters

Pathology does not always appear directly in front of the capsule.

Angioectasias, erosions, ulcers, inflammatory changes, polyps, and other abnormalities may be located along the bowel wall or partially concealed behind mucosal folds. A lateral panoramic view may therefore provide clinicians with a more direct perspective of the surface where these abnormalities occur.

The goal is not simply to capture more images.

It is to capture more clinically meaningful perspectives.

A Wire-Free Examination Experience

CapsoCam Plus stores examination images within the capsule itself. As a result, patients do not need to wear an external data recorder, sensor belt, leads, or adhesive pads during the procedure.

This wire-free design can help simplify the examination experience by allowing patients to continue much of their normal daily routine without carrying external recording equipment.

It can also reduce several operational steps for healthcare teams:

·        No recorder fitting before capsule ingestion

·        No recorder availability limitations

·        No end-of-day return of wearable equipment

·        No charging, cleaning, or maintenance of recording belts

·        No radiofrequency transmission during image acquisition

The onboard storage design also eliminates the risk of radiofrequency interference during data transmission and supports examination in patients with certain implantable electronic devices, subject to the device instructions for use and the physician’s clinical assessment.

From Ingestion to Image Review

The CapsoCam Plus workflow is designed around four straightforward stages:

1. Capsule ingestion

The patient swallows the capsule according to the preparation and procedural instructions provided by the healthcare team.

2. Capsule collection

After natural excretion, the capsule is recovered using the specially designed CapsoRetrieve® collection system.

3. Data download

The retrieved capsule is placed into the CapsoAccess® system, which extracts the stored examination images.

4. Clinical review

The physician reviews the captured images using CapsoView® software, which provides panoramic viewing, playback controls, image adjustment tools, suspected-bleeding detection support, and report-generation functions.

This streamlined process is designed to reduce equipment-related scheduling bottlenecks and minimize the time healthcare professionals spend preparing, fitting, collecting, and maintaining external recorders.

Designed Around Both Clinical and Patient Needs

Capsule endoscopy is more than an imaging technology. It is also a patient journey.

By combining panoramic visualization with a wire-free examination, CapsoCam Plus brings together two important priorities:

Comprehensive small-bowel imaging for physicians and greater freedom and convenience for patients.

Patients are not required to carry a recorder throughout the examination or return to the clinic simply to remove external equipment. Capsule retrieval also provides confirmation that the capsule has been excreted.

For hospitals and clinics, the absence of dedicated wearable recorders can support more flexible scheduling and reduce dependence on the availability of reusable equipment.

Seeing Beyond the Scope

Innovation in capsule endoscopy is not only about making devices smaller or capturing images faster.

It is about improving where the cameras look, how much mucosa clinicians can visualize, and how easily the examination fits into clinical practice and the patient’s day.

With four lateral cameras, complete panoramic imaging, onboard data storage, and a wire-free workflow, CapsoCam Plus offers a different perspective on small-bowel evaluation.

Because when every visible detail may contribute to a clinical decision, perspective matters.
